This secure portal is used to complete all required documentation prior to your appointment at Beauty Labs. If this is your first time logging in, your username is the email address used when booking your appointment. A temporary password was sent to you via email or text message by our team. You may reset your password once logged in.
Please use the portal to complete the following prior to your visit:
Botulinum Toxin Consent Form
​This form is required and must be completed annually before any neurotoxin treatment.
Annual Medical Evaluation with a Licensed Provider
A virtual telehealth consultation is required once per year prior to receiving any botulinum toxin injections. This must be completed and documented in your chart before treatment.
Chart Access
Review your treatment notes, before and after images, and other documentation following each appointment.
IV Hydration & Vitamin Injection Forms
If receiving IV therapy or intramuscular vitamin injections, required consents and health history forms are available within the portal.
Please ensure all documentation is completed before your scheduled appointment to avoid delays or rescheduling.
